Social Work



Social Workers interact with the patient, family and rehabilitation team (including insurers) and can assist in the following ways:


  • Evaluate the patient’s total living situation, including lifestyle, family finances, employment history, and community resources, and assess the impact of disability or disease on these areas.
  • Maintain a continuing relationship with the patient and family.
  • Coordinate funding resources and discuss financial arrangements and concerns. This may include a case management role.
  • Help the family develop the skills needed to participate actively in treatment procedures in the home.
  • Provide assistance in locating alternative living situations.
  • Assess vocational barriers.
  • Provide emotional support to patient and family in stressful situations.
  • Facilitate discharge planning.
